To prevent bacteria growth, and the potential for spreading communicable disease, liquid pump soap is needed for hand washing. *Correct by August 25/25.* - A storage shelf was observed in one of the toilet stalls in the washroom used by the children. To prevent tipping and the potential for injury to the children, the shelf must be sturdy and safe. *Correct by August 25/25.* Adequate sleeping arrangements for the age and development of the children in the proposed program were available.

    Observation. During the review of the daily attendance log in the Preschool program, it was observed that one child had not been signed in. This issue was highlighted to the Licensee during the inspection. The child had been transferred from another program earlier in the day. Attendance records must reflect sign in and sign out times for children moving back and forth between both programs. The Licensee addressed and corrected the record while the inspection was in progress.

    Observation. - The Licensing Officer reviewed a sample of staff files and some were missing evidence of character reference checks. *This contravention was noted on the last routine inspection report. In addition, a staff person working in the facility must have a criminal record check completed through the Criminal Records Review Program in Victoria. A RCMP criminal record check is not accepted as it does not have an expiry date.

  • C7.1 - Is the facility safe, sanitary, free from hazards, in good repair, and appropriate for the age and development of children?Non-critical

    Observation. - The Licensing Officer observed some expired supplies in the first aid kit. *Resolved during the inspection. During the inspection, the Licensee informed the Licensing Officer that the heating system in the Preschool program room was currently being repaired. As a result, there were portable heaters in place. The Licensing Officer observed that these portable heaters were not accessible to the children in care.

  • C4.1 - Are facility records current and complete?Non-critical

    Observation. - The child emergency records were missing emergency contacts other than the parents. This information must be readily available to the staff when the children are taken off site and when parents cannot be reached in the event of an emergency. The Licensing Officer and the Licensee discussed adding more detailed information to the minor incident records. This record should include, but is not limited to, any first aid treatment that the staff may have provided to the child, the time that the minor incident occurred, how and when the parent was informed, and a signature or initials of the staff person who created the record in the minor incident log. - The Licensee did not have a record keeping system in place for recording any medication that may be administered to a child in care. This record must include the name of the child, the name of the medication, the amount of medication that was administered (dosage), the time the medication was administered, and the name of the person who administered the medication. During the inspection, the Licensing Officer reviewed the Licensee's other record keeping systems such as the daily attendance records and the play space inspection log, etc. and they appeared satisfactory at this time.

    Observation. - As per the large scale evacuation plan, ensure that the emergency kit contains an adequate amount of supplies for the capacity of children in care and that the supplies are complete and not expired.

    Observation. - Ensure child records include personal information such as doctor/clinic contact information. - Child emergency records must include pertinent personal information such as care card numbers and the sex of the child which is an identifying feature. - Ensure any care plans in place for a child include the required information such as the date of creation and evidence that the parent was involved in the creation of the care plan for the child. - Any additional information, revisions to the care plan, etc. must be documented in the child's care plan as per what is required in Section 58 of the Child Care Licensing Regulation. Licensing reviewed other record keeping systems, such as minor incident logs and play space inspection logs. These record keeping systems appeared satisfactory at the time of the inspection.

Based on the information and documentation gathered by Licensing during this complaint investigation, the Licensing Officer determined that the following allegation was substantiated: • Children were attending the Preschool (30 Months to School Age) and Group Child Care (30 Months to School Age) programs who were younger than 30 months of age. - RESOLVED - This contravention was previously identified on the complaint inspection report dated January 12, 2024 and the follow-up inspection report dated January 26, 2024. A compliance plan was submitted by the Licensee and accepted by Licensing on February 7, 2024. This plan sufficiently addresses this recurring contravention.

    Observation. Children were attending the Preschool (30 Months to School Age) and Group Child Care (30 Months to School Age) programs who were younger than 30 months of age. - Information regarding the correction of this contravention was included in the Licensee's immediate written Health and Safety plan. - Observed by Licensing that the Licensee was ADHERING to the immediate written Health and Safety plan regarding this issue at the time of this inspection. At the start time of this inspection, there were 7 children present in the Group Child Care (30 Months to School Age) program room with 1 ECE and 2 ECEA staff. • At the start time of this inspection, there were 14 children present in the Preschool program with 4 ECEA staff. One ECEA staff person was replacing the qualified ECE staff who is required in the Preschool program. This lesser qualified staff person (ECEA) has been working in the role of the required ECE since January 4, 2024. This staff replacement exceeds the allowable 30 consecutive days. Also, as per Section 34 (2)(b) and Schedule E of the Child Care Licensing Regulation, a minimum of one qualified ECE staff and one qualified ECEA are required to meet the Preschool program's staff to child ratio (i.e. with a program capacity of 20).

    Observation. • At the time of this inspection, an Early Childhood Education Assistant (ECEA) staff person was replacing the qualified Early Childhood Educator (ECE) who is required for the staff to child ratio in the Preschool program. This lesser qualified staff person (ECEA) has been working in the role of the required ECE since January 4, 2024. This staff replacement exceeds the allowable 30 consecutive days.

    Observation. Based on the information gathered by Licensing during the complaint follow-up, the Licensing Officer determined that the following allegation was substantiated: • Some children attending the Group Child Care (30 Months to School Age) program were placed in high chairs as a disciplinary response to the child's behaviour. Specifically, when a child was observed to be touching or taking another child's food, the child exhibiting this behaviour was then placed in a high chair. Licensing explained to the Licensee that anything that restricts a child from having freedom of movement and is used in response to a child's behaviour is considered restraint and must not be used unless the requirements specific to the use of restraint in the Child Care Licensing Regulation (CCLR) have been met. The Licensee confirmed to Licensing that there were no care plans currently in place for any of the children in care. *Information regarding the Licensee's plan of correction to address this contravention was received by Licensing on January 27, 2024. While this plan of correction stated how the Licensee achieved immediate compliance, Licensing has requested that the Licensee revise this plan to include the system(s) that the Licensee has implemented to monitor ongoing compliance (i.e. specific to the use of restraint). Licensing acknowledged that the Licensee and staff were also using the high chairs appropriately with children who demonstrated that they needed extra support with sitting safely while eating and while doing certain crafts.

    Observation. • Children were attending the Preschool (30 Months to School Age) and Group Child Care (30 Months to School Age) programs who were younger than 30 months of age. - Information regarding the correction of this contravention was included in the Licensee's immediate written Health and Safety plan. - Observed by Licensing that the Licensee was NOT ADHERING to the immediate written Health and Safety plan regarding this issue at the time of this inspection. During the inspection, the Licensee stated that they wanted to rescind their immediate written Health and Safety plan currently in place. Licensing stated that an accepted Health and Safety plan was required to be in place and adhered to by the Licensee in order to mitigate the immediate risks to the children in care that had been identified by Licensing. Licensing advised the Licensee that they could rescind their plan or revise it, however, Licensing still required an accepted Health and Safety plan to be put in place by the Licensee to address the immediate risks. Licensing informed the Licensee that a plan put in place that would contravene any section of the Child Care Licensing Regulation would not be accepted.

    Observation. • A system for ensuring that furniture is cleaned in a manner that minimizes the potential spread of illness was needed. - Observed by Licensing to be RESOLVED at the time of this inspection. • A high storage shelf in the Preschool room must be anchored to prevent tipping. - Observed by Licensing to be RESOLVED at the time of this inspection. • A second exit in the Preschool room was blocked with children's boots. - Observed by Licensing to be RESOLVED at the time of this inspection. • Some of the interconnected rubber floor tiles had unsecured pieces that created a tripping hazard. An unsecured floor rug in the Group Child Care (30 Months to School Age) program bathroom was a slip hazard. - Observed by Licensing to be RESOLVED at the time of this inspection. • Other than two playpens, Licensing did not observe any age appropriate sleeping arrangements available for the children who fall asleep in the Preschool room. - Observed by Licensing to be RESOLVED at the time of this inspection. • Hazardous products, such as cleaning products, nail polish remover, bingo dabbers, and shaving cream were found accessible to the children in care in the Group Child Care (30 Months to School Age) and Preschool program rooms. - Observed by Licensing to be RESOLVED at the time of this inspection. • The laundry closet bi-fold door was observed to be off its track and was leaning up against the stackable washer and dryer. - Observed by Licensing to be RESOLVED at the time of this inspection. • A system for monitoring the fridge temperature was needed in the Preschool program room. - Observed by Licensing to be RESOLVED at the time of this inspection. Observed by Licensing to be UNRESOLVED - Ensure that a 6 foot clearance is maintained around equipment, such as slides, to provide space for the children in care to safely enter and exit the equipment. The child records that were previously identified on the January 12, 2024 inspection to be missing required information, such as doctor/clinic contacts, were not reviewed by Licensing during this inspection. The Licensee confirmed with Licensing in their compliance plan dated January 25, 2024 that this contravention had been resolved. Licensing will verify this correction at a future follow-up inspection.

    Observation. Based on the information gathered by Licensing during the inspection, the Licensing Officer determined that the following allegation was substantiated: • Children in care in the Group Child Care (30 Months to School Age) program were observed playing with small Lego toys which are a choking hazard. Licensing observed some children put the Lego repeatedly in their mouths and the staff instructed these children to take these toys out of their mouths. - This requires immediate correction. It should be noted that Licensing observed six high chairs available in the Group Child Care (30 Months to School Age) program. When Licensing asked why six high chairs were needed for the children attending the program, the staff verbally confirmed that the chairs were used with some of the younger children for eating and art activities.

    Observation. During the inspection, the Licensing Officer identified the following additional contravention: • Children were attending the Preschool (30 Months to School Age) and Group Child Care (30 Months to School Age) programs who were younger than 30 months of age. *Information regarding the correction of this contravention was requested to be included in the submission of the immediate Health and Safety plan.* • More than two children between the ages of 30 and 36 months were attending the Group Child Care (30 Months to School Age) program. As per Section 2 (1)(c) "Care programs" of the Child Care Licensing Regulation (CCLR), a Preschool (30 Months to School Age) program, is defined as a program that provides care to preschool children who are at least 30 months old on entrance to the program and 36 months old by December 31 of the year of entrance. As per Schedule E of the Child Care Licensing Regulation (CCLR), a Group Child Care (30 Months to School Age) program is only permitted to have no more than 2 children in care between the ages of 30 and 36 months at one time.

  • C3.1 - Does the facility have sufficient employees who possess adequate training and experience to meet the care, supervision, and needs of the children?Non-critical

    Observation. Based on the information gathered by Licensing during the inspection, the Licensing Officer determined that the following allegations were substantiated: • Some staff files were missing required documents (i.e. evidence of work experience). • Children attending the Group Child Care (30 Months to School Age) program, the Preschool program, and the Group Child Care (School Age) program were grouped together for longer than half an hour at the beginning of the day and half and hour at the end of the day. A Preschool program is not permitted to be grouped together with any other care programs as per Section 34 (4)(a) of the Child Care Licensing Regulation (CCLR). During the inspection, the Licensing Officer identified the following additional contravention: • Persons over the age of 12 years were ordinarily present in the facility while children were in care without complete and valid criminal record checks from the Criminal Records Review Program. *Information regarding the correction of this contravention was requested to be included in the submission of the immediate Health and Safety plan.*

During the inspection, the Licensing Officer identified the following additional contravention: • Children attending the Preschool (30 Months to School Age) program were in care for longer than 4 hours each day. - This requires immediate correction. *It should be noted that the allegation named in the complaint regarding the Licensee not complying with the Child Care Licensing Regulation, Section 52 (1)(b) - "Harmful actions not permitted" is still under investigation by Licensing and a determination had not been made at the time of this inspection.

    Observation. Based on the information gathered by Licensing during the inspection, the Licensing Officer determined that the following allegations were substantiated: • In preparation for children to eat, the tables were not wiped down and cleaned first before they were sprayed with a disinfectant by the staff. A system for ensuring that furniture is cleaned in a manner that minimizes the potential spread of illness appears to be needed. • A high storage shelf in the Preschool room must be anchored to prevent tipping. • A second exit in the Preschool room was blocked with children's boots. - This requires immediate correction. • Some of the interconnected rubber floor tiles had unsecured pieces that created a tripping hazard. An unsecured floor rug in the Group Child Care (30 Months to School Age) program bathroom was a slip hazard. • Staff verbally confirmed with Licensing that children attending the Preschool program lie down for rest time next to each other on a rug that is directly on the floor. Blankets and pillows were available for the children to choose from. Other than two playpens, Licensing did not observe any age appropriate sleeping arrangements available for the children who fall asleep in the Preschool room. • Hazardous products, such as cleaning products, nail polish remover, bingo dabbers, and shaving cream were found accessible to the children in care in the Group Child Care (30 Months to School Age) and Preschool program rooms. All of these items stated "Keep out of reach of children" on their labels. - This requires immediate correction. • The laundry closet bi-fold door was observed to be off its track and was leaning up against the stackable washer and dryer. During the inspection, the Licensing Officer identified the following additional contraventions: • A system for monitoring the fridge temperature was needed in the Preschool program room. The fridge temperature must not exceed 4C to ensure safe food storage and prevent food borne illness. • Ensure that a 6 foot clearance is maintained around equipment, such as slides, to provide space for the children in care to safely enter and exit the equipment.

Based on the information gathered by Licensing during the inspection, the Licensing Officer determined that the following allegation was substantiated: • Daily attendance records were incomplete. The weekly attendance sheet for the Preschool program (Side B) was kept in the Group Child Care (30 Months to School Age) program room (Side A). During the inspection, the Licensing Officer identified the following additional contraventions: • Some child records were missing doctor or medical clinic contact information. • A file with completed documents for a volunteer was not available on the premises for Licensing to review. • Child records for some of the children in care were not available on the premises for Licensing to review. *Information regarding the correction of this contravention was requested to be included in the submission of the immediate Health and Safety plan.* A staff person working in the Group Child Care (30 Months to School Age) program verbally confirmed with Licensing that there were no children attending the program who required extra support and, therefore, no care plans were currently in place.

As a result of this investigation, it was discovered that the staff were not following the facility's own policies in which children must wear a pinny or a wrist band that identifies the facility when out in public spaces. There are no emergency cards or photographs of the children than can be readily available when in the community Some Children's records are missing physician's information Enrollment dates missing from three children's files. During today's inspection it was observed that today's attendance records were incomplete. In Room 119, none of the 5 children present had their attendance recorded. In Room 121, only 7 out of 12 children present had their attendance recorded.
