  Observation. Reportable incident described in daily log reports that during an outing a child walked away and when the child did not return when called, the responsible adult "hid behind the trees and waited for him to follow us."

  Observation. Reportable incident described in daily log reports that during an outing a child walked away from the caregiver and when the child did not come when called, the caregiver "hid behind the trees and waited for him to follow us."

  Liquid hand soap and single use towels are not readily available at all hand wash sinks.

  Observation. Licensee has provided facial tissue for children to dry hands.

  Observation. Diaper changing has been occurring in the main activity room not adjacent to a washbasin.

  Observation. Children under 36 months have not be using a separate sleep area but instead have been napping on a couch in the activity area.

  Observation. Scented air freshener (manufacturers warning "keep out of reach of children") was found in an accessible place in washroom and was moved to a high shelf during inspection. Hot water tank and cleaning products in hallway closet with unsecured bifold door are accessible by children.

  Observation. Reportable incident described in daily log reports that during an outing a child walked away from the husband of Licensee who was providing care as a responsible adult. When child would not comply with his directions, the RA hid behind a tree to see if child would follow.
