  Observation. A large, deep puddle remains on the playground due to non-functioning drains. Staff confirm the water does not drain.

  Observation. Corners on the half-wall (between cubbies and main room are damaged from wear and tear and the drywall corner beads are exposed and sharp. Wallpaper is also peeling off.

  Observation. Staff informed Licensing Officer that during rainy days water collects in play yard. The puddle that forms is over four inches deep and covers a large area, about 1/3 of play area. The water does not drain and is standing until evaporated.
