Licensing observed that an EpiPen prescribed for a specific child was stored in a storage room rather than being kept in a location where it was immediately accessible to staff in the event of an emergency. Licensing also observed that the EpiPen remained inside the classroom while the child was outdoors during outdoor play.

Also, the child’s care plan on file was signed in January 2025. Licensing observed some arrival and departure times are not being captured.

Observation. Children's files were available and organized, but some were missing medical insurance information. It was also observed that information that was missing on emergency card was found in the registration paperwork or via versa. The concern with this is, if staff only took emergency cards in emergency not all information may be available. While staff person was observed signing children in, there was a period where not all children were signed in as they arrived, it is important that the sign in form is accurate at all times.

Observation. At today's inspection - LO reviewed files and emergency cards, which have not been updated since last visit. As noted below, files continue to be missing - immunization, emergency contact, information given by parent, medical insurance plan number, doctor's contact and permission for emergency care. Manager stated that she will be reviewing the files and updating the forms to use one single type moving forward. LO will return early summer to review files. As noted March 14, 2023 - LO reviewed 17 children's files. 3 appear complete with the remainder either missing Immunization record, emergency contact, record of any relevant into given by the parent or medical insurance plan number. It was also noted that there are various types of forms used, which can make it more confusing to ensure all information is in the file. An inspection checklist that includes children's files checklist is included with this report, and is recommended to be used when reviewing files. It is also recommended that all paperwork be kept in a single location with all required documentation that licensing will review each year.

It was also observed that a child's permission to administer medication form was mixed in with staff files.

Observation. This is a recurring contravention - the outdoor space has items such as garbage bag, metal and wood that appears to be used for building. While the indoor area has shelving and equipment that is in need of repair. LO recommends using checklist to ensure each area is clean and free from hazards on a daily basis.

Observation. LO observed the following: -in the outdoor space, a stack of high chairs which appear to be dirty and soiled. -chipped paint on the purple and blue shelving units in the classroom. The shelving units appears to be in poor repair and no longer easy to clean. -facility walls in the classroom space appear to be stained and soiled leaving a visible residue. The walls are not clean and no longer easy to clean.
