  Observation. Licensing reviewed 10 of 10 staff files. One file was missing proof of 20 hours of training for Responsible Adult status.

  Corrective action. Ensure a responsible adult has completed 20 hours of training in child development, guidance, health & safety or nutrition.

  Observation. Licensing observed an incident recorded in a staff file that occurred at the facility on January 28, 2022 that was not reported to Licensing.

  Corrective action. Ensure an MHO is notified within 24 hours if a child has been involved in/may have been involved in a reportable incident.
