  Observation. Licensing audited 5 children's files and observed that 1 was missing a record of persons not permitted.

  Observation. Licensing audited 5 children's files and observed that 2 files were missing MSP numbers, 1 was missing sex, and 1 was missing immunization status.

  Corrective action. Ensure to keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.

  Observation. Licensing reviewed attendance records and observed that times of departure were missing for dates in August, September, and October 2022.

  Corrective action. Ensure that, for each child, there is a record showing times of arrival and departure.

  Observation. Licensing reviewed four of four children's records. One child's record was missing a signed copy of the repayment agreement.
