    Observation. CCFL reviewed staff files and compared them to staff lists provided by the facilities. Several incomplete and missing files were observed and some staff members listed in the provided staffing list did not have a file.

    Observation. LO noted the following - the lid on one of the toilet's tank located in the children's bathroom was damaged and had some tape on it. The damage is posing a risk for injuries - no thermometer was available in the freezer to monitor the temperature
