  Observation. Licensing observed that the composite play structure was dismantled and is no longer a risk to the health and safety of children.

• A licensee must do all of the following:(b) operate the community care facility in a manner that will promote the health, safety and dignity of persons in care

  Observation. Licensing conducted a file review prior to the inspection and observed that the last facility policies were dated September 2016. During the inspection, the License stated that the policies had not been updated. Additionally, there was no written plan and procedures for Communicable Disease Prevention.

  Corrective action. Ensure that the facility is operated in a manner that promotes the health, safety and dignity of persons in care, including current policies for Communicable Disease Prevention.

  3060 - CCALA 7(1)(b)(i)

• For equipment over 45 cm (18”) but less than 1.5 metres (5’), protective surfacing is recommended

  Observation. Licensing observed that the fall height of the composite play structure was 24" (61 cm) tall. The depth of the protective surfacing material (i.e. pea gravel) was tested in multiple locations in the fall zone and measured between 0 - 3" in depth.

  Corrective action. Ensure that equipment over 18" (45 cm) fall height has adequate depth of protective surfacing.

• 3000 - SPS DOLSOP (Entanglement)

  Observation. Licensing observed that there was entanglement hazards on the top left and right hand sides of the slide attached to the composite climber. This represents an immediate risk to the health and safety of persons in care and must be corrected immediately. The Licensee stated that the slide would be closed effectively immediately, and the entire play structure would be disassembled over the coming long weekend. Licensing accepted this plan. Full compliance will not be achieved until the plan has been implemented and completed.

  Corrective action. Ensure all outdoor play materials accessible to children are safely constructed and free from hazards.

• A licensee must keep current records of each of the following: (f) a log of minor accidents, illnesses and unexpected events involving children, that did not require medical attention and were not reportable incidents described in Schedule H.

  Observation. Licensing observed a minor incident log with the most recent entry dated July/2022. Licensee stated that while minor incidents had occurred since July/2022, they had not been recorded.

  Corrective action. Ensure a licensee must keep current records of each of the following: (f) a log of minor incidents, illnesses and unexpected events involving children, that did not require medical attention and were not reportable incidents described in Schedule H.

• A licensee must keep, for each child, a record showing the following information: (a)name, sex, date of birth, medical insurance plan number (MSP number does not apply to REC only) and immunization status.

  Observation. Licensing audited five of nine children's files. Four files were missing an MSP number.

  Corrective action. Ensure a licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.
