    Observation. Licensing conducted a file review prior to the inspection and observed that the last facility policies were dated September 2016. During the inspection, the License stated that the policies had not been updated. Additionally, there was no written plan and procedures for Communicable Disease Prevention.

    Corrective action. Ensure that the facility is operated in a manner that promotes the health, safety and dignity of persons in care, including current policies for Communicable Disease Prevention.

    Observation. Licensing observed that the fall height of the composite play structure was 24" (61 cm) tall. The depth of the protective surfacing material (i.e. pea gravel) was tested in multiple locations in the fall zone and measured between 0 - 3" in depth.

    Corrective action. Ensure that equipment over 18" (45 cm) fall height has adequate depth of protective surfacing.

    Observation. Licensing observed that there was entanglement hazards on the top left and right hand sides of the slide attached to the composite climber. This represents an immediate risk to the health and safety of persons in care and must be corrected immediately. The Licensee stated that the slide would be closed effectively immediately, and the entire play structure would be disassembled over the coming long weekend. Licensing accepted this plan. Full compliance will not be achieved until the plan has been implemented and completed.

    Corrective action. Ensure all outdoor play materials accessible to children are safely constructed and free from hazards.
