    Observation. Through the investigation process, Licensing determined that the parents of a child involved in a reportable incident were not immediately notified of the incident.

    Corrective action. Ensure that parents of a child who has been or may have been involved in a reportable incident are notified immediately. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to this contravention. Action/s Required by Licensing: On January 22, 2025, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: Parents or emergency contacts would be notified immediately by phone or Kindertales app if their child was involved in a reportable incident. The corrective action plan was reviewed and accepted by Licensing.

  • A licensee must ensure that a child is not, while under the care or supervision of the licensee, subjected to any of the following types of abuse or neglect: physical abuse

    Observation. Through the investigation process, Licensing determined that a staff had physically abused a child by placing her hand over a child's mouth to stop the child from screaming.

    Corrective action. Ensure the community care facility is operated in a manner that will promote the health, safety and dignity of persons in care, specifically regarding staff not physically abusing a child. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to this contravention. Action/s Required by Licensing: On January 22, 2025, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: the staff involved in this incident had been terminated on December 19, 2024. New employees were provided with all facility policies and direct observation of new staff related to behavioral guidance would be performed by management. The corrective action plan was reviewed and accepted by Licensing.

    Corrective action. Ensure all staff follow the Behavioral Guidance Policy at the facility. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to this contravention. Action/s Required by Licensing: On January 22, 2025, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: All staff would review the Behavioral Guidance Policy and Child Abuse Policy by January 27, 2025, and on January 30, 2025, all staff would attend a mandatory staff meeting to discuss guidance policies. The corrective action plan was reviewed and accepted by Licensing.
