  Observation. There is sharp edged roofing exposed on a wood box in the outdoor play area. There are discarded items piled in the outdoor play area, such as broken equipment and empty bottles.

  Observation. The staff did not have access or knowledge of the facility COVID-19 safety policy or the emergency plan.

  Observation. There was no emergency plan on site that the staff knew about. The emergency food on-site expired in 2016 and the emergency water expired in 2015. The bag indicates that it came from 'nursery'.

  Observation. There are no registration records on-site for the children in care. The staff could only produce copies of the emergency cards.
