Observation. The diaper change mat in the Under 36 months program appears stained and not in good repair. It is important for such equipment to be clean and well maintained as it allows it to be properly cleaned and sanitized. Submit a compliance plan on or before November 17, 2025 to address this issue.

Observation. Upon review of the minor incident log, it was determined that a reportable incident occurred that was not reported to Licensing. Incidents that fall under the definitions in Schedule H of the CCLR must be reported to Licensing within 24 hours. Submit a compliance plan on or before November 20, 2025 to address this issue.

Observation. Three employees have criminal record checks on file that were not obtained through the licensee's organization. Criminal record checks from other organizations are not transferable and must be shared through the criminal records review program.

Observation. Several issues noted in one of the children's washrooms: a soiled diaper left on the change table, urine left in the potty training toilet, cloth cover on change table which is not easily cleaned between uses.

Observation. Upon arrival to the inspection employee to child ratios were not in place as outlined in Schedule E in each program. This was corrected during inspection.

Observation. There was no evidence on file that a person providing support services at the facility has a criminal record check, references and immunization through their employer.

Observation. One child that requires extra support does not have a care plan in place.

Observation. The facility has not been tested for radon.

Observation. Immunization information was missing from three employee records. 1 employee does not have a valid criminal record check (CRC). The previous CRC expired September 2022.
