    Observation. A criminal record check for a person over the age of 12 who is ordinarily present was expired.

    Corrective action. Licensee said she will obtain a criminal record check for the resident. Licensee said the resident does not have direct access to children.

    A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

    Observation. Playpen used for sleep has multiple holes throughout the mesh siding.

    Corrective action. The licensee said a the playpen will be replaced with a cot or another playpen, which are available at the facility.

    Observation. Licensing observed the following accessible to children: - An electrical outlet cover was broken in the gross motor area, with a sharp edge - Stacked blankets and sheets were located beside a playpen used for nap with the potential to fall - Cleaning products located under the bathroom sink

    Corrective action. Licensee said all items will be addressed today.

    Observation. Licensee said minor incidents are verbally told to parents/guardians, however they are not recorded.

    Corrective action. Licensee said the incidents will be recorded moving forward and a log book is availalbe for use.

    Observation. The emergency exit to the front door was blocked with items. The Licensee stated they will be moved.

Observation (CORRECTED DURING INSPECTION): Lysol wipes were accessible to children under the bathroom sink. The Licensee made the wipes inaccessible at time of inspections.

    Corrective action. Ensure that hazardous items are inaccessible to children in care.

    A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

    Observation. The following was observed downstairs: -toys were stored on the top of a tall shelf with the potential to fall. -a tall shelf is unstable and moves when pushed on. -a tile towards the emergency exit at the front door is broken and moves when stepped on. -Clothes were hanging on a rack beside a play pen used for sleeping with the potential for the child to pull the clothes down. The following was observed upstairs where children were at time of arrival: -the laminate flooring at the top of the stairs was loose and posed a tripping hazard. -Items and clothes were piled high beside a couch where children were sitting with the concern that the items might fall on children. -Dog feces were observed in the outdoor play area.

    Corrective action. Address the concerns above to insure a healthy and safe environment.
