A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

Observation. A plug was observed to have burned in an infant toddler program. A hole in the wall in an infant toddler room covered by a paper. chipped paint and dirty walls were observed in many program rooms Bathroom door and some nap doors are off the track and difficult to open

Corrective action. ensure that furniture, equipment and fixtures are clean and in good repair.

Without limiting subsection (1), a licensee must ensure that (a) children attending a community care facility are divided into groups, with each group having no more children than that permitted in Schedule E.

Observation. In more than one group Child Care (30 months - school age) there were more than 2 children under the age of 36 months.

Corrective action. Ensure that there are no more than 2 children under the age of 36 months in the group child care (30 months- school age)

A licensee must do all of the following: (b) operate the community care facility in a manner that will promote (i) the health, safety and dignity of persons in care,

Observation. Licensing observed that the bathroom toilets were visible from the classroom and children using them were not provided privacy.

Corrective action. The licensee will provide a written response to Licensing detailing their action plan to address the contravention.

A licensee must supply equipment, furniture and supplies that are (b) located so as not to block or hamper an exit in the case of fire or other emergency.

Observation. The emergency gate from the infant toddler playground was unable to be opened.

Corrective action. Ensure that the emergency exit is able to be accessed in case of emergency.

A licensee must ensure that any heated water accessible to children is heated to no more than 49° Celsius.

Observation. Water temperature in Preschool Age room 1 was greater than 49 degrees Celsius.

Corrective action. Ensure that water temperature is heated to no more than 49 degrees Celsius.

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

Observation. The following was observed: - Lysol was located in the hallway bathroom -brooms and mops were being stored in bathrooms next to toilets -blind cords were hanging posing a strangulation hazard in more than one room -items were stored on top of cupboards and were hanging over the edge with the potential to fall -mirrors were observed in bathrooms that allow children to see other children when using the toilet. A discussion was had with the Manager regarding the dignity and privacy of children. In the outdoor play area -a new climbing dome was added. there were wood rounds located at the bottom posing a hazard. -bikes were available to children without any safety equipment such as helmets. DOLSOP Education and injury prevention for sports safety. On the outdoor patio the children use with staff: -milk crates were located next the railing which is about 4 feet tall. There is concern that the children could stand on the milk crates and fall over the railing which has a 9 foot drop. -a small portable BBQ with a small bottle of propane attached to it was on a table that was in poor condition and accessible when children are using the space. The Manager said it was used for a children's camping activity. -broken toys and boxes were located in this shared area.

Corrective action. Ensure that the facility is providing healthy and safe environment.

A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

Observation. The following was observed: - Walls in most classrooms were in poor repair with holes or chips in the walls -nap beds in more than one program were beginning to come apart at the corners and unable to be properly sanitized -diaper mats in more than one program were beginning to come apart and unable to be sanitized -tables and shelves appeared dirty

Corrective action. Ensure that the facility, and equipment is clean and in good repair.

A licensee, other than a licensee who provides a care program described as either Preschool (30 Months to School Age) or Group Child Care (School Age), must have in a community care facility a sturdy surface for changing diapers, located outside the food preparation area, next to (a) a covered container for soiled clothing.

Observation. One program did not have a sturdy surface for changing diapers and 2 programs did not have a covered container for soiled clothing.

Corrective action. Ensure each program has a sturdy surface for changing diapers next to a covered container.

A licensee must display in a prominent place in the community care facility the certificate, or a copy of the certificate, of any educator or assistant employed to work in the community care facility.

Observation. Two educator or assistant certificates were not posted.

Corrective action. Ensure each educator or assistant certificate is posted in a prominent place.

If a licensee has agreed with a parent to give a child any medication prescribed by a medical practitioner or provided by the parent, the licensee and his or her employees must ensure that the medication is (b) readily accessible to employees.

Observation. Medication was not readily accessible to employees.

Corrective action. Ensure that medication is readily accessible

Without limiting subsection (1), a licensee must ensure that (a) children attending a community care facility are divided into groups, with each group having no more children than that permitted in Schedule E.

Observation. A child in the Infant Toddler 1 program was over the age of 36 months. A conversation was had with the Manager that the child would require a retention to stay in the program.

Corrective action. Ensure that children are divided according to Schedule E.

A licensee must keep, for each child, a record showing the following information: (h) any special instruction respecting the child's diet, medication, participation in a program of activities, or other matter relevant to the child's care (ii) agreed to by the licensee.

Observation. The facility did not have the name of a medication that was requested to be given to the child.

Corrective action. Ensure that the name of the medication is obtained

A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.

Observation. One child did not have the name or telephone number of a medical practitioner.

Corrective action. Ensure each child has a record of the name and telephone number of a medical practitioner.

A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.

Observation. One child did not have a medical insurance plan number.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (f) evidence that the person has complied with the Province's immunization and tuberculosis control programs. Observation (CORRECTED DURING INSPECTION): Immunization status was not obtained for one employee. This was corrected at time of inspection

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (c) a record of the person's work history.

Observation. One staff person's work history was not obtained .

Corrective action. Ensure a record of staff persons work history is obtained.

A licensee must not employ a person in a community care facility as (a) an educator or an assistant unless the person holds a certificate issued under Division 2 [Employee Qualifications], which must be verified by the system maintained by the early childhood educator registry. (Educator or assistant certificate validation) Observation (CORRECTED DURING INSPECTION): One staff person did not have their certificate verified. It was verified while licensing was on site.

Observation. An employee did not have a criminal record check obtained by the employer.

Observation. At the inspection licensing observed 2 care plans that were not reviewed yearly. There was also 1 care plan without a photo and 1 without a date.

Corrective action. Please ensure care plans are reviewed yearly and are dated, signed and contain a photo of the child.

A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.

Observation. Review of 8 children's files noted 1 missing a medical practitioner name and phone number.

Corrective action. Please ensure children's records are complete, parents may provide a walk in clinic.

Observation. Review of 8 children's files noted 1 did not have a medical insurance plan number and 3 did not have immunization status.

Observation. The exit to the playground has a lock located at the top of the gate above the reach of some staff. Staff at the time of inspection were unable to reach the lock. The staff explained that they have to climb on the ledge or bring something to stand on to open the gate.

Corrective action. Ensure that the emergency exit is accessible to all staff.

Observation. It was observed that the second staff person in a Group Child care (under 36 months) program was an Early Childhood assistant. It was discussed with the Manager about the ratio for different service types.
