  • C1.1 - Does the licensee continually inform licensing of any significant changes to the structure or operation of the facility?Critical

    Observation. - Two educator certificates were not posted in the facility. To ensure transparency and visibility, the Licensee must display the certificates of any educator or educator assistant employed to work in the facility. *Correct by: August 7, 2025.* The facility licence and the name of the current Manager were posted in the facility as required.

  • C7.2 - Is there adequate and appropriate indoor and outdoor space, materials, and equipment?Critical

    Observation. - A step stool was not available in the washroom by the elevators. To promote independent hand washing, ensure that a step stool is provided in the washrooms that are designated for use by the children in care. *Correct by August 13, 2025.* - The bolts on a rebounder that secure the handle to the base of the equipment appeared to be loose. To prevent potential injury to the children in care, ensure that the rebounder equipment is safely constructed and in good repair. *Correct by: August 7, 2025.*

  • C7.1 - Is the facility safe, sanitary, free from hazards, in good repair, and appropriate for the age and development of children?Non-critical

    Observation. - Both the kitchen and the laundry room located in the common entry area, and that stored hazardous cleaning products, were found open. The children in care in all of the programs use this common area as a pathway to the outdoor play yard and, therefore, had access to these two rooms. To prevent accidental poisoning, hazardous cleaning products must be made inaccessible to the children in care. *Correct by: August 6, 2025*. - Medicated diaper creams were found in drawers and were accessible to the children in care. To prevent accidental swallowing and to avoid contact with eyes, these creams must be made inaccessible to the children in care. *Correct by: August 6, 2025*. - Small objects such as staples, paper clips, and a package of batteries were found in a drawer and were accessible to the children under 36 months old. To prevent accidental swallowing and/or choking, these small objects must be made inaccessible to the children in care. *Correct by: August 6, 2025*.

  • C4.1 - Are facility records current and complete?Non-critical

    Observation. - Some of the personal information in the children's emergency records, such as care card numbers and doctor/clinic contact information, was missing. To ensure that the staff have immediate access to the children's personal information while taking the children off site and/or in the event of an emergency or evacuation, this information must be complete and up to date. *Correct by: August 13, 2025.* Other records such as the daily attendance, minor incident logs, and the play space inspection log were reviewed and they appeared satisfactory.

  • C2.1 - Are written policies and procedures in place to guide employees in all matters regarding the care and supervision of children?Non-critical

    Observation. - The monthly fire drill was not practiced in July 2025. Practising and recording the fire drills monthly shows the plan's effectiveness of its implementation thereby minimizing the risk to children in the event of a fire. *Correct by: August 13, 2025.* - As per the emergency evacuation plan for the facility, activities for the children were not available in the evacuation kit. Having these supplies readily available will help to minimize the physical and emotional impact on the children during an emergency. *Correct by: August 13, 2025.* - In addition, the first aid kits were missing supplies such as triangular bandages. In order for the staff to respond to any injury sustained by a child in care, adequate supplies must be readily available. *Correct by: August 13, 2025.* Other than the written procedure for creating and formatting child care plans, the Licensee has not revised or updated any other policies and procedures since the last inspection.

  • C10.2 - Are children supervised, and are employee to children ratios maintained?Critical

    Observation. The additional contravention identified below was not included in the complaint allegations received by Licensing on July 1/25. - A school age child was observed in the facility and who was under the care and supervision of the Licensee. The contact for the Licensee verbally confirmed that there were 5 school age children present in the facility and who were attending an "unlicensed school age summer camp" in one of the classrooms that was licensed for a Group Child Care (Under 36 Months) program with a capacity of 12. The Licensee is not licensed to operate a Group Child Care (School Age) program in the facility nor can school age children be cared for in the room that is currently licensed for a Group Child Care (Under 36 Months) program. The Licensee had previously informed Licensing that this classroom was not in use due to flooring repairs and that the affected Group Child Care (Under 36 Months) program was not operating out of that room while the repairs were being done. The contact for the Licensee stated that the flooring repairs had been completed, and rather than resuming operation of the Group Child Care (Under 36 Months) program that was licensed for that space, the Licensee decided to operate an unlicensed school age summer camp. This "summer camp" started operating on July 7/25. It was explained that the entire facility is licensed and, therefore, an unlicensed program could not operate in the licensed room unless the designated Group Child Care (Under 36 Months) program operating out of that room was removed from the licence and the room was designated as "unlicensed space" on the facility floor plan. Caring for school age children in a licensed Group Child Care (Under 36 Months) program is a contravention to Section 2.1(1)(a) of the CCLR. *Confirmation of this contravention being resolved is required by: July 10, 2025.* In the “Lakeshore Learners” room, licensed for a Group Child Care (30 Months to School Age) program, there were 12 children with 1 Infant and Toddler Educator (ITE)and 1 Early Childhood Educator Assistant (ECEA) staff, covering for an absent Early Childhood Educator (ECE), present. The children were participating in nap/rest time. With regards to the allegations specific to behaviour guidance and harmful actions, additional information will be gathered by conducting interviews with specific employees of the Licensee. These interviews will be conducted by Licensing on July 16/25.

  • C4.1 - Are facility records current and complete?Non-critical

    Observation. Based on the information gathered, the investigating LO has determined that the following allegation is substantiated: CCLR, Section 58(3)(d) - Care plans - Substantiated. Allegation: A care plan in place for a child to address a challenging behaviour did not appear to be working as the behaviour continued to be exhibited by the child. - A care plan in place for a child requiring extra support was reviewed. The care plan was originally created in 2024. The care plan was reviewed and signed by the new facility Manager in 2025. There was no evidence that the actual content of this care plan had been updated or revised since its creation in 2024. There was no evidence in the care plan that showed the Licensee had recorded compliance with the behavioural guidance provided to the child, and its effect, since the original creation date of the plan. The relevant minor incident log was reviewed and which showed that the child with the care plan, had exhibited the same challenging behaviour on 8 separate occasions over a 9 week period starting in April 2025. By not recording the effect of the behaviour guidance in the child's care plan, the Licensee's staff will continue to implement the same behaviour guidance techniques as outlined in the plan. Since the behaviour continued to be exhibited by the child on multiple occasions, it is apparent that some or all of the guidance techniques being used by the staff were not having the desired positive effect. Copies of relevant child records, such as a care plan and specific minor incident documents, were requested to be submitted as evidence to Licensing by July 10/25. *Confirmation of this contravention being resolved is required by: July 16, 2025.*
