  Observation. The name of the manager is not displayed. The managers name must be displayed to identify the person responsible for the program. Submit to Licensing by November 25, 2025, evidence the manager's name has been displayed in a prominent place.

  Observation. No evidence that an annual emergency plan has been practiced. Practicing emergency plans ensure facility preparedness during an emergency and the health and safety of children in care. Submit to Licensing by November 27, 2025 confirmation the required annual emergency plan has been practiced.

  Observation. Child files incomplete missing medical insurance number, medical practitioner name, number and date of enrollment. Incomplete child files may put children at risk during an emergency. Submit to Licensing by November 25, 2025, written confirmation child files are complete.

  Observation. The Licensee did not send a reportable incident for "service delivery problems" to Licensing. Child registration records were reviewed. One child's registration record was missing the name and contact details of an emergency contact, other than the child's parent.
