  Observation. It was observed that two sharp nails were pointing out of a play house and there were broken plastic pieces within outdoor play space accessible to children. A chipped area on a ride-on toy was identified, posing a potential safety hazard. Although outdoor play checks are recorded, a thorough check is not being conducted. Ensure that the play area is properly maintained prior to children using the space and that this information is accurately recorded. Ensure that all the indoor and outdoor materials and equipment are free from hazards, and in good repair. A rubber pad cover on the edge of the fireplace hearth was missing. Another side of the pad cover was observed to broken and hanging from one end, posing a potential safety hazard. Children were observed to sit and read books in this area. Ensure that a healthy and safe environment is provided at all times. The diaper change mat was torn. Ensure that equipment is easy to clean and free from hazards. It was observed that hazardous items that indicate "keep out of reach of children" such as hair spray, anti-cavity fluoride rinse and light bulbs were found accessible in the children's bathroom cabinet. Ensure that children do not have access to these hazardous items. The heated water in the children's washroom is 53.2 degrees Celsius. Ensure that any accessible heated water to children is heated to no more than 49 degrees Celsius.
