A licensee must ensure that the entire outdoor play area is (a) enclosed in a manner that is suitable for the age and development of children, and will ensure that children are free of harm.
Observation. Licensing observed the following: - Space between stairs outside in Daycare playground is big enough for a child to get through. This area has exposed nails that are accesible to the children. - Entrapment hazard in the Daycare playground under the fence
Corrective action. The manager will make sure the outdoor area is safe from hazards

A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.
Observation. Cleaning supplies and lysol wipes being kept in unlocked cabinet in School Age program
Corrective action. The manager will ensure the supplies are locked

A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.
Observation. Licensing observed three fire extinguishers were not serviced. The food and water in the emergency supplies was expired
Corrective action. The manager will follow up about servicing the fire extinguishers and replace the food and water with non-expired items.

A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.
Observation. Licensing observed that children are not being signed in/out of classrooms when children move classrooms in the morning
Corrective action. The manager will ensure the attendance records are completed.

The licensee must (b) review the care plan at least once each year with a parent of the child requiring extra support and any person requested by the parent.
Observation. The care plan did not have a date or a parent signature indicating it was reviewed for the current year.
Corrective action. Staff said that the care plan was reviewed with the parent but she did not remember a date of when it occurred and said she will obtain a date and signature.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.
Observation. One staff did not have a valid criminal record check on file.
Corrective action. Manager said that it had been applied for in September; however, no record was provided.

A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. Upon inspection there were 4 playpens used for napping that had visible holes in the mesh siding and there was a large patch of exposed gyprock in the classroom that had residue that came off on fingers when touched.
Corrective action. Please ensure the facility and equipment is in good repair while children are in attendance.

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. Upon inspection there were: - large picture frames leaning up against the wall, above the cubbies that posed a fall hazard. - a large, heavy, glass globe on a high shelf that posed a fall hazard in the event of an earthquake.
Corrective action. Please ensure a safe environment is provided at all times.

Observation. Upon inspection 1 staff did not have her certificate to practice posted.

Observation. Upon inspection, there were medicated diaper creme's on site but no medication forms permitting staff to apply them to the children.
Corrective action. Please ensure medication forms are completed by the families.

Observation. Upon inspection, the children's records were unable to be produced. Staff stated that the registration's are kept in the office but nobody on site at the time of the inspection, had access to it.

Observation. A review of the checklist for children's files indicates that photos are not available for two children.
Corrective action. Please ensure that a photo is available for all children. The Licensee Contact stated that missing photos of children will be taken on the facility camera and printed.
