The licensee must (b) review the care plan at least once each year with a parent of the child requiring extra support and any person requested by the parent.
Observation. Review of three Care plans, of those two had not been reviewed annually with the parent of the child.
Corrective action. Manager said they will be reviewing the care plans with parents next week.

- EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. In the bathroom two mirrors were rusted, with the screws holding the mirror in place were rusty and sticking out of the wall accessible to children, and making it difficult to sanitize.
Corrective action. Please ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair.

A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.
Observation. 12 children's records were reviewed. 6 children's records were missing the immunization status.
Corrective action. Please ensure to keep records showing immunization status.

Observation. Licensing observed the following: - A sofa which showed sign of wear and tear. - Some chipping paint on one of the facility doors.
Corrective action. Ensure that the community care facility's furniture and fixtures are in good repair.

A licensee must keep current records of each of the following: (c) a record respecting compliance with section 22 (2) (b) and (c) [emergency training and equipment].
Observation. Licensing observed that a fire drill was not recorded for the month of October. A discussion with the Manager indicated that a fire drill had been practiced but not recorded.
Corrective action. Ensure fire drills are recorded/logged once conducted.

A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.
Observation. upon inspection of 6 children's records, Licensing observed: - 3 children's records were missing medical practitioner information. -1 child's record was missing an emergency contact.
Corrective action. Ensure children's records are complete at the time of enrollment.

A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure. Observation (CORRECTED DURING INSPECTION): Upon licensing's arrival at the facility, it was noted that only 5 of the 16 children in attendance were signed in with a time of arrival.
Corrective action. Ensure that times of arrival and departure are accurately documented.

Observation. a person who does not have training for a responsible adult was working as the second staff in ratio.
Corrective action. Ensure that staff to child ratios are no less staff than permitted in Schedule E

In respect of a record referred to in this Division, a licensee must (a) keep in a single place at the community care facility the records referred to in sections 56 to 58.
Observation. Staff files for multiple staff who worked at the facility this week were not available for review. There is a staff checklist that was last updated in October 2017 that does not have all staff written on it.
Corrective action. Ensure that all staff files are kept in a single location at the facility.

A licensee must keep, for each child requiring extra support, a current care plan showing the following information: (b) the courses of action recommended by health care professionals to address the needs of the child requiring extra support.
Observation. 1 care plan was observed that did not have the course of action recommended to address the needs of the child.
Corrective action. Ensure the course of action to address the child's needs is on the care plan.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (d) copies of any diplomas, certificates or other evidence of the person's training and skills.
