  Observation. Many of the walls are damaged and require repair and painting.

  Observation. Many of the emergency supplies had expired, including, food, water, light sticks.

  Observation. The only staff in the program had no qualifications. Over the phone the manager informed licensing that an employee unexpectedly discontinued working at the facility.

  Observation. The staff file only contained immunization status.
