  Observation. There is a hole through the rubber tiles where a stump was removed that creates a tripping hazard. The rubber tiles are starting to curl up on the uneven surface.

  Observation. The playground is located in a snow load fall zone making it unusable for much of the winter.

  Observation. The rubber tiles in the outdoor play space are lifting and the playground surface is uneven due to large tree roots. The playground is closed much of the winter due to snow load hazard from the roof.

  Observation. The outdoor rubber tile surfacing is coming up at the edges because of tree roots and causing a tripping hazard. There is a shelf in the activity space that is over 4' and not anchored to the wall.

  Observation. One staff file was missing a certificate verification.
