    Observation. Regarding intake May 23, 2025. Through the investigation process, Licensing determined that staff picked up and carried a child against their will to move the child from the outdoor area to the indoor area.

    Corrective action. Ensure that a child, while under the care or supervision of the Licensee, is not subjected to confinement or physical restraint by an employee, except as authorized in a child care plan if the plan includes instructions respecting behavioral guidance. Action/s Required by Licensee: Licensee to submit a Corrective Action Plan to Licensing specific to the contravention. Action/s Required by Licensing: On May 28, 2025, Licensee submitted a Corrective Action Plan to Licensing. The Corrective Action Plan included additional staff training and staff review of facility policies. The corrective action plan was reviewed and accepted by Licensing.

Licensing observed the following outside: Rusty metal nails inside a plastic toy house, metal hinges protruding from the outdoor fence, exposed sharp metal edges on a white tube.
