  Observation. Licensing audited four children's file. One file was missing the name and telephone number of medical practitioner.

  Corrective action. Ensure s record is kept for each child containing the name and telephone number of medical practitioner.

  Corrective action. Ensure a criminal record check is obtained for all persons over the age of 12 that are ordinarily present.
