  Observation. Licensing observed a log of all inspections, maintenance and repairs was not kept since September 2025 in Preschool (30 Months to School Age) program, and the log was not kept since January 2026 at Group Child Care (30 Months to School Age) program and School Age on School Grounds Program.

  Corrective action. Ensure a log of all inspectons, maintenance and repairs is kept.

  Observation. Licensing observed the attendance was not kept on March 13, 2026.

  Corrective action. Ensure a record of daily attendance is kept.

  Observation. Licensing observed the recording of fire drill was missing since March 2025, and the yearly emergency plan practice was missing since October 2024. The manager stated they had practiced both fire drill and emergency drill, but did not record.

  Corrective action. Ensure a record is kept of monthly fire drill and yearly emergency plan practice.

  Observation. Manager stated that the Group Child Care (School Age) program had practiced an emergency drill in October 2021, but did not record it. This item was corrected at time of inspection.

  Corrective action. Ensure a record is kept of yearly emergency plans practiced.

  Observation. Licensing observed two medication forms. Two forms did not indicate signs and symptoms of when to administer the medication.

  Corrective action. Ensure medication is administered in the times and amounts specified in the child's record, including signs and symptoms of when to administer.
