    Observation. Licensing observed a log of all inspections, maintenance and repairs was not kept since September 2025 in Preschool (30 Months to School Age) program, and the log was not kept since January 2026 at Group Child Care (30 Months to School Age) program and School Age on School Grounds Program.

    Corrective action. Ensure a log of all inspectons, maintenance and repairs is kept.

    Observation. Licensing observed the attendance was not kept on March 13, 2026.

    Corrective action. Ensure a record is kept of monthly fire drill and yearly emergency plan practice.
