  Observation. Licensing observed that children were sent to the washroom in a buddy system and with no staff supervision. The washrooms were located down a hall and in the foyer of the church accessible by the public.

  Corrective action. Ensure record are kept for each child showing written consent to call an ambulance.

  Observation. Licensing audited six children files, two files were missing records of immunization status for children in care.

  Corrective action. Ensure a records of children's immunization status is kept.
