  Observation. Licensing observed a log of all inspections, maintenance and repairs was not kept since March 2026. Licensee stated they inspected the playground before the children in use, but did not recorded.

  Corrective action. Ensure a log of all inspections, maintenance and repairs is kept.

• A licensee must do all of the following:(b) operate the community care facility in a manner that will promote the health, safety and dignity of persons in care

  Observation. Licensing observed a corrective action plan for lead in water was not followed. Manager stated they flushed the water for two minutes but did not record it. Manager stated children did not drink water from the bathroom tap, and they refilled children's water bottle from kitchen tap.

  Corrective action. Ensure to operate the community care facility in a manner that will promote the health, safety & dignity of persons in care.

• A licensee must ensure that safe drinking water is available to children.

  Observation. Licensee stated they refilled children's water bottle from kitchen tap. Licensing observed the facility had no record of lead in water test result from the kitchen tap.

  Corrective action. Ensure safe drinking water is available to children.

  3050 - CCLR 48(5)
• A licensee must keep, for each child, a record showing the following information: (a)name, sex, date of birth, medical insurance plan number (MSP number does not apply to REC only) and immunization status.

  Observation. Licensing audited six children's file. One file was missing the record of immunization status, and two files were missing the record of medical insurance number.

  Corrective action. Ensure to obtain the missing immunization record and medical insurance number for all child's file.

• A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner (doctor's name and number does not apply to REC only) and emergency contact

  Observation. Licensing audited six children files. Two files did not contain the name and telephone number for a medical practitioner.

  Corrective action. Ensure a record is kept for each child showing the name and telephone number for a medical practitioner.

• A licensee must keep, for each child, a record showing the following information:(i) a photograph or digital image of the child, and other information that can be used to readily identify the child in an emergency

  Observation. Licensing audited six children files, and observed six missing photos to readily identify child.

  Corrective action. Ensure all children files had evidence of photos of child that can be used to readily .
