    Observation. - The diaper changing mat has developed tears and is in damaged condition. It is important to ensure that the diaper changing mat is free from any tears or damages as compromised surfaces can-not be effectively cleaned and sanitized, increasing the risk of contamination and the spread of illness. Submit a response with evidence of compliance by April 07, 2026. - Bags containing dog feces are accessible to children in the outdoor play space. Having hazardous items within children's reach poses a potential health and safety risk to children. This was corrected during inspection.
