  The facility, the furniture, equipment, fixtures and supplies are of sturdy and safe construction.  They are easy to clean, and are clean and in good repair and free from hazards. [CCLR Sec. 13(2) and Sec. 15(1)(a)]

  Observation. Licensing observed the following: * floors and bathrooms were visibly soiled; a discussion occurred with the Manager about the cleanliness of the spaces while children are in care * one of the stall doors in the girls' bathroom (hallway bathroom) is loose and may fall if forced to close; this was brought to the attention of the Program Supervisor and a staff * caulking around the toilet bases in the boys' bathroom was inconsistent which may cause damage to the flooring and therefore cleaning the surfaces in this location is less effective

  Licensing is notified within 24 hours after a child is involved in, or may have been involved in, a reportable incident described in Schedule H or has a reportable communicable disease. [CCLR Sec.55(2)(a)(b)]

  Observation. An altercation was documented in a logbook. It involved two children and resulted in a staff moving a child. Based on the details provided, this physical redirection meets the definition of an emergency restraint. This incident although reported to the children's parents, was not reported to Licensing. "emergency restraint", which means a restraint that is necessary to protect the child or others from imminent physical harm that is not approved and documented in a child's care plan.
