  Observation. Items identified in routine inspection needing repair or replacement have not been attended to.

  Observation. Two epi-pens provided for child with severe allergy are expired.

  Observation. Licensing observed Epi-pen in outside pocket of facility backpack which was stored on the floor in children's cubby area.

  Observation. Licensing observed club chair in the texemay program space to be in a state of disrepair. Upholstery has disintegrated and peeled making chair difficult to clean. Children's faucet in xixits'ay program space is not in proper working order, water only dribbles out of faucet. The condition of the outdoor fencing and gate used by children has deteriorated due to exposure by sun, wind and salt (facility is oceanside).

  Observation. Facility management has instructed staff to record all conversations concerning children's health and well-being in facility logbooks; however, a system ensuring all staff are aware of the records has not been established.
