  Observation. LO reviewed 3 children's files and noted: • 1 child does not have an emergency contact listed - parents have provided their own contact information • 1 child does not have a PHN/Care Card number on file • 1 child does not have immunization status on file

  The certificates of early childhood educators and assistants, or copies of certificates, are prominently displayed in the facility. [CCLR 11(4)]

  Observation. LO observed that a staff working as an ECEA did not have her certificate displayed in the facility.

• PHYSICAL, EQUIPMENT AND FURNISHING / EnvironmentNon-critical

  The facility, the furniture, equipment, fixtures and supplies are of sturdy and safe construction.  They are easy to clean, and are clean and in good repair and free from hazards. [CCLR Sec. 13(2) and Sec. 15(1)(a)]

  Observation. LO observed that the washroom mirror appeared soiled and in need of cleaning. LO observed that the toilet appeared soiled and in need of cleaning. LO observed that the diaper changing mat is ripped in 2 spots. The mat is no longer in good repair and easy to clean.

• POLICIES AND PROCEDURES / Emergency Training and EquipmentNon-critical

  There is an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency. [CCLR Sec. 22(1)(b)]

  Observation. LO observed that emergency supplies kept on site did not contain water.

  There is current log of minor incidents, illnesses and unexpected events, that did not require medical attention and were not reportable incidents described in Schedule H. [CCLR Sec. 56(1)(f)]

  Observation. LO did not observe that a log book is kept on site.

  Evidence of compliance with the Province's immunization and tuberculosis control programs has not been obtained. [CCLR Sec.19(1)(f)]; Character references have not been obtained. [CCLR Sec.19(1)(b)]; Copies of any diplomas, certificates or other evidence of the person's training and skills have not been obtained. [CCLR Sec. 19(1)(d)]; Records of staff's work histories have not been obtained. [CCLR Sec. 19(1)(c)]; Criminal record checks have not been obtained. [CCLR Sec. 19(1)(a)]

  Observation. LO observed that one staff working as a Responsible Adult did not have documentation on file. LO observed that one staff working as an ECEA did not have documentation on file, however was able to produce documents that she had stored on her phone.

  There is evidence of employees' continued compliance with the Province's immunization and tuberculosis control programs. [CCLR Sec 21(1)]

  Observation. LO did not observe immunization status on file for a staff working as a Responsible Adult.

  Fire drills are not practiced at least once each month. [CCLR Sec. 22(2)(b)]

  Observation. LO did not observe evidence that fire drills were conducted over the past year.

• CARE AND/OR SUPERVISION / Group Sizes and Employee to Children RatiosNon-critical

  The group size(s) exceeds that permitted in Schedule E. [CCLR Sec. 34(2)(a)]

  Observation. LO observed that there were 3 children in care under the age of 24 months. Family Child Care allows for 4 children younger than 48 months old, with no more than 2 of those children younger than 24 months old.

  Observation. LO observed that 3 children did not have immunization status on file, and one child did not have a medical insurance plan number on file.

  Hazardous objects and substances are inaccessible to children. [CCLR Sec 17]

  Observation. In the bathroom there is a Lysol can that is under the sink, accessible to children.

  A record for each child containing the daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure is not kept. [CCLR Sec. 57(2)(c)]; A record for each child containing the name, sex, date of birth, medical insurance plan number and immunization status is not kept. [CCLR Sec. 57(2)(a)]

  Observation. Attendance was not completed for the past few days, and does not include time in/out. Licensee stated that parents never remember to complete it. LO reminded Licensee that she can fill it out. It was also observed that two children are missing medical insurance plan number.

• STAFFING / Additional criminal record checksNon-critical

  All persons over the age of 12 that are ordinarily present on the premises while children are present are of good character and the licensee has obtained a criminal record check for each person. [CCLR Sec 20]

  Observation. CRC's for all persons living in the home over the age of 12 have expired, and there is currently no CRC for staff person. I will share information on how to apply for a CRC in the email with this report.
