  Observation. Licensing observed the care plan for a child in the program had expired.

  A record for each child containing the daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure is not kept. [CCLR Sec. 57(2)(c)]; A record for each child containing any notification of a parent, emergency contact or medical health officer made under CCLR section 55 [notification of illness or injury] is not kept. [CCLR Sec. 57(2)(g)]; A record for each child containing the date of enrolment in the facility is not kept. [CCLR Sec. 57(2)(b)]

  Observation. Licensing observed when going through the current records for each child, it was noted that there were children's files that were not up to date missing start dates, medical practitioner contact information, emergency card not filled out. etc. Licensing also observed the program was not documenting children's departures when the child left the program for the day.

[CCLR Sec. 57(2)(d)]; A record for each child containing the name, sex, date of birth, medical insurance plan number and immunization status is not kept. [CCLR Sec. 57(2)(a)]

  Observation. The following was observed: -date of enrolment is not kept for 10 children -emergency contact is not kept for 1 child -medical insurance plan number is not kept for 1 child

  Observation. LO observed that the criminal record check for 1 staff is addressed to another facility LO observed that the criminal record check for 1 staff expired on February 4, 2021
