    Observation. A volunteer who is present in the facility does not have character references. Incomplete screening of volunteers may increase risk to children. Submit to licensing by May 19, 2026, evidence of having obtained character references for the volunteer. Some staff files are missing character references, certificate validation, one staff has an expired criminal record clearance letter. Incomplete staff files reduce transparency and weaken oversight which can put children at risk. Submit to licensing by May 19 2026, evidence the missing items are in the staff files. Additionally, submit a plan that outlines how the staff files will be maintained to ensure the required documents are collected and kept current.

    Observation. The outdoor wooden stairs located in the Group Child Care (Under 36 Months) outdoor play space is accessible to children. The stairs pose a risk of falls and injuries to young children. Submit to licensing by May 19, 2026, evidence the stairs have been made inaccessible to the children.

    Observation. Two staff files are incomplete. Items missing included character references, immunization status and educator certificate validation. Staff records must be complete and maintained for the entire time a person is employed. Submit evidence the missing information is contained in the staff files.

    Observation. One care plan has not been reviewed yearly. Licensing requires each care plan to be reviewed yearly to ensure all the information is up to date and relevant. Submit evidence of having reviewed and dated the care plan. Play space inspections were observed to be incomplete for the weeks of April 21 - 25, 2025, April 28 - May 2, 2025, May 5 - May 8, 2025. Play space inspection must be completed daily to ensure the environment is safe for the children. Submit a plan to licensing that demonstrates how play space inspections are conducted and recorded each day.

    Observation. It was observed the Group Child Care (30 Months to School Age) program, and the Group Child Care (School Age) programs have access to a crawl space located underneath the stairs in the lower level of the facility. This area poses a potential risk to children as the visibility of the area is limited. Submit to licensing evidence that children do not have access to the area underneath the stairs in the lower level of the facility A review of the facilities emergency equipment was conducted during the inspection. All the food items in the emergency supplies are outdated. Insufficient emergency equipment and supplies pose a potential risk to children. Submit a plan to licensing that demonstrates how the emergency equipment will be maintained.

The wooden Tee Pee play structure located in the Group Child Care (30 Month to School Age) program is in poor repair. Play space logs have not been maintained. The protective resilient surfacing in the Group Child Care (30 Months to School Age) program requires replenishing in order to meet the standards as per the Director of Licensing Standards of Practice-Safe Play Space.

    Observation. The loose rubber chips fill material is inadequate in the Group Child Care (30 Months to School Age) outdoor play yard. Depth guidelines for loose fill material for 5 foot equipment height is a minimum of 6 inches . The loose fill material was completely missing in areas around the climbing structure. In some spots the loose fill was not at the recommended 6 inches.
