Observation. The managers name was not posted in a prominent place in the facility. This was resolved during the inspection.

Observation. The facilities minor incident log books were reviewed during the inspection. The log books did not contain information regarding how or when the parent was notified of illness or injury. A licensee must immediately notify a parent or emergency contact if a child becomes ill or injured while in care. Submit to licensing by July 17, 2025, a plan that details how staff will ensure a written record is kept of notifying parents of illness and injuries. One child file did not contain a consent to release the child to someone other than the parent in case of accident, illness, or other emergency if the parent cannot immediately be reached. An incomplete child file may put the child at risk in an emergency situation. Submit to licensing by July 17, 2025, evidence the missing information has been collected. A review of the facilities daily play space inspections was conducted. The play space inspections were found to be incomplete in the Group Child Care (30 Months to School Age) program. Incomplete play space inspections may increase risk to children. Submit to licensing by July 17, 2025, a plan that addresses how staff will ensure the play space inspections are conducted and recorded consistently.
