    Observation. Regarding intake dated March 24, 2026. Through the investigation process, Licensing determined that no documentation was noted regarding the child prior to the reported incident.

    Corrective action. Ensure to keep current records of each of the following: (f) a log of minor accidents, illnesses and unexpected events involving children, that did not require medical attention and were not reportable incidents described in Schedule H. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On April 20, 2026, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: Minor incident logbook to maintain proper documentation of incidents would be used with parent/guardian signing off on upon notification by staff. Routine checks of incident documentation by management to ensure proper notification of events to Licensing, initialing upon completion. The corrective action plan was reviewed and accepted by Licensing.
