  Observation. There is one child who requires an Epi Pen and Puffer, care plan / medication form was not complete nor was there adequate information available.

  Observation. Not all children had complete files.

  Observation. One staff did not have a complete file on record and as such was missing the immunization status.

  Observation. When the van arrived there were 9 children and one adult - the driver.
