  Observation. Licensing observed that various children's emergency consent card to have outdated pictures. The facility should have current pictures of each child either printed or digitally.

  Observation. Licensing reviewed staff documentation and observed required documentation had not been obtained.

  Observation. Licensing noted 10 children's were in care of 1 RA staff. The staff `s first aid certification was expired.
