    Observation. LO observed that one couch was in poor condition and could not be properly cleaned and sanitized because the vinyl surface was damaged, exposing the porous material underneath.

    Observation. Licensing observed the fire drill for November 2024, December 2024, and January 2025 were not recorded. Manager stated that she had performed fire drill, but had not recorded.

    Corrective action. Ensure a current record is kept for all monthly fire drill.
