  Observation. There are many toys that are broken, missing wheels and parts, sharp edges on broken plastic and debris in the playground space. The Manager shared that new sand is due to be added to the sand box shortly.

  The facility, the furniture, equipment, fixtures and supplies are of sturdy and safe construction.  They are easy to clean, and are clean and in good repair and free from hazards. [CCLR Sec. 13(2) and Sec. 15(1)(a)]

  Observation. Children's beds / mats had small tears and cracks present. In each of the rooms there are several upholstered fabric couches that do not have a removable cover for cleaning.

  Observation. There is pea gravel over the entire playground. There are areas that have a very shallow covering, exposing hard packed ground and also making the surface very slippery.

  For each child requiring extra support, the care plan does not show the courses of action recommended by health care professionals to address the needs of the child requiring extra support. [CCLR Sec.58(1)(b)] ; Care plans are not reviewed at least once a year with a parent of the child requiring extra support and any person requested by the parent. [CCLR Sec. 58(2)(b)]

  Observation. There are two children with care plans that need to be updated. Medication was not adequately labelled.
