  The facility, the furniture, equipment, fixtures and supplies are of sturdy and safe construction.  They are easy to clean, and are clean and in good repair and free from hazards. [CCLR Sec. 13(2) and Sec. 15(1)(a)]

  Observation. There is an upholstered fabric couch in the room. There is a mattress on the floor with a sheet cover that is washed every three weeks. There are large stuffed animals that are washed every three weeks. There is a cord to the stereo in the nap room that is accessible to the crib adjacent to the outlet.

  Observation. This play ground has pea gravel in all areas. There are areas where the coverage has shifted and minimally covers a hard pack surface. This creates not only an impact hazard should a child fall but a slipping hazard as the ground is very slippery where there is minimal gravel.
