Observation. Licensing observed a mattress added to a pack and play. Manufacturer instruction state not to add any mattress. Licensing also observed outdoor check list was last completed July 4, 2025. Licensee stated she's completed the check each morning but hadn't recorded them.
Corrective action. Ensure to operate a facility in a manner that will promote the health, safety and dignity of persons in care.
