  Observation. Licensing reviewed six child files and observed one file missing a Medical Services plan number and missing record of the child's immunization status and/or immunization records.

  Corrective action. Ensure to keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.

  Observation. Licesning reviewed six child files and observed one file missing record of the child's Medical Practitioner name and telephone number.

  Corrective action. Ensure to keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.

• A licensee must ensure that each employee practices implementing the emergency plan at least once each year.

  Observation. Licensing observed the monthly fire and yearly fire drill record log, since being licensed, had not contained record of a yearly emergency drill and Licensing observed staff state a yearly emergency drill had not been conducted. No yearly emergency drill had been conducted in the past twelve months.

  Corrective action. Ensure that each employee practices implementing the emergency plan at least once each year.

  Observation. Licensing observed no record of daily outdoor play space checks recorded for the month of December 2024 and up to the date of inspection, January 8, 2025.
