Observation. The facility was licensed in July 2025. Testing for radon gas for this facility, which is located in the lower level of the building, has not been completed as required. These policies and procedures are accepted by Licensing. Fire safety equipment is inspected annually and is in good repair. Monthly fire drills are practiced and displayed as required.

Ensuring that parents have also given consent for emergency contacts to pick up their child, will help to guide staff when an emergency with a child in care arises and to avoid confusion. *Correct by: June 30, 2026.* - Some of the children's records do not list identifying features such as eye colour and the photos of these children, that can be used to readily identify the child in the event of an emergency, do not clearly show the child's eye colour. Ensure that identifying features of children in care are either clearly shown in a photo and/or is documented. *Correct by: June 11, 2026.* Other records such as the minor incident logs, daily attendance records, and play space inspection logs are complete as required and appeared satisfactory.

Observation. Allegation: The Licensee did not have an adequate system in place for ensuring that the outside play spaces are safe and thoroughly inspected (i.e. daily, weekly, and monthly) for any hazards which is a contravention to the Director of Licensing Standards of Practice - Safe Play Space, Maintenance of Equipment. Findings: Substantiated. Based on the information gathered during the inspection, the Licensing Officer determined that the Licensee's system for inspecting the outdoor play spaces used by the children in care was inadequate and did not show how the play spaces were thoroughly checked by the staff for a variety of potential health and safety hazards that may be present daily, weekly and/or monthly. For example, checking the fencing for any needed repairs, checking the placement of equipment to ensure a clearance of 6 ft is maintained around the equipment, checking to see if any accessible debris found naturally in the yard (such as sticks and logs) pose a potential risk to the children in care. etc. In addition, the Licensing Officer and the manager discussed how the staff in each program were informed if any hazards were found during the outdoor play space inspections and if the shared play space would need to be temporarily closed for maintenance/repairs. *The correction date deadline for this contravention is November 26, 2024.*

Observation. During the inspection, the Licensing Officer requested and reviewed the Licensee's systems for logging minor incidents involving children in care, how parents were notified of these minor incidents, and the systems for recording outdoor play space inspections. Allegation: The Licensee did not record a minor incident involving a child in care which is a contravention to Section 56(f) of the Child Care Licensing Regulation (CCLR). Findings: Substantiated. Based on the information gathered during the inspection, the Licensing Officer determined that a minor incident that occurred on November 14, 2024, which involved a child who was under the care and supervision of the Licensee, was not recorded in the facility minor incident log. *The correction date deadline for this contravention is November 22, 2024.* Allegation: The Licensee did not have an adequate system in place for checking that the outside play spaces are safe and free from hazards which is a contravention to the Director of Licensing Standards of Practice - Safe Play Space, Maintenance of Equipment. Findings: Substantiated. Based on the information gathered during the inspection, the Licensing Officer determined that the Licensee's record keeping system for the outdoor play space inspections was inadequate. The Licensee's outdoor play space inspection log showed minimal information and did not show how the play spaces were thoroughly checked for a variety of potential health and safety hazards that may be present daily, weekly and/or monthly. *The correction date deadline for this contravention is November 26, 2024.* Allegation: The Licensee did not immediately inform the parent of a child who was involved in an incident while under the care and supervision of the Licensee which is a contravention to Section 55(1)(a) of the CCLR. Findings: Unsubstantiated. Based on the information gathered during the inspection, the Licensing Officer determined that the Licensee notified the parent of the child in care who was involved in a "minor" incident that occurred at the facility on November 14, 2024. A "minor" incident is described as an incident that did not require medical attention and was not a reportable incident to Licensing as defined in Schedule H of the CCLR. The term "injury" is not defined in the legislation, therefore, the Licensee has some discretion as to when to notify the parent of a child who has been involved in a "minor" incident unless otherwise instructed by the parent.

- C1.1 - Does the licensee continually inform licensing of any significant changes to the structure or operation of the facility?Critical
Observation. - During the inspection, the Licensing Officer observed construction occurring in the lower level of the building for the amendment to the facility licence that is currently in progress. The current licensed programs are operated out of the upper level of the building and do not have access to the lower level. However, the Licensing Officer observed that the construction had impacted the use of the outside play yard primarily used by the Preschool program. Part of the fencing had been removed, a large piece of construction equipment was present in the yard, and a large hole was dug up. The Licensing Officer was not notified that a change to the outside play space would occur nor how the health and safety of the children would be maintained during the construction and closure of this space. The Licensing Officer requested the manager submit a written health and safety plan as well as an outside play rotation schedule as the Preschool program is now sharing the other outside spaces with the Group Child Care (30 Months to School Age) program but not at the same time. - The Early Childhood Education Assistant (ECEA) certificate for a staff person working in the facility today was not displayed.

Observation. - The Licensing Officer reviewed a sample of staff files and some were missing evidence of their immunization status.

Observation. - A covered container for soiled items was not available as required in the Group Child Care (30 Months to School Age) program with a capacity of 16. *This contravention was noted on the last routine inspection report. The Licensing Officer discussed current cleaning practices in place at the facility with the manager. While the program staff clean and sanitize surfaces as needed throughout the day, a cleaning/janitorial crew completes other cleaning duties. The Licensing Officer observed adequate hand washing supplies in place.

Observation. - Ensure that medicated diaper creams are kept inaccessible to the children in care. - Ensure that a separate sleeping area is provided for a child who is younger than 36 mos of age and who is attending the Group Child Care (30 Mos to School Age) program.* *It is important to note that during the initial inspection on June 23, 2021, it was observed that a separate room designated on the floor plan as a "nap room" for the Group (30 mos to School Age) program was being used as a storage room. As a result, there was not enough inside space for the applied capacity of 25 and only a maximum of 23 children could attend this program at one time. The licensee then changed the storage room back into a nap room in order to meet the 3.7m2 of useable space requirement per child for the program capacity of 25 and to have the licence issued as per the application. During the inspection today however, licensing observed that this designated "nap room" had been turned back into a storage room. The manager informed licensing that as a result of this, they were ensuring that their program capacity did not exceed 23 children at one time. During the inspection, licensing observed 19 children in care in the Group (30 mos to School Age) program. Licensing made it clear to the manager that if they have more than 23 children in care at one time in the Group (30 Mos to School Age) program then the storage room must be changed back into a nap room in order to be included as useable space.*
