  • C2.1 - Are written policies and procedures in place to guide employees in all matters regarding the care and supervision of children?Non-critical

    Observation. - Based on the information gathered, the facility’s written policy/procedure for minor incidents involving children in care is not followed by the staff. This policy states that “All Minor Injury Reports MUST be signed by the parent/guardian at pick-up and then stored in the child’s file located in the office.” Some minor incident records involving children in care are not signed by the parent of the child involved in the incident. In some instances, it is confirmed that the staff only verbally informed the parent of the incident involving their child. The legislation states that policies and procedures, intended to guide staff in the care and supervision of children, must be implemented by employees. *This contravention is not listed as one of the allegations in the complaint.* *Correct by: June 24, 2026.* - Based on the information gathered, a written action plan to help guide staff in the care and supervision of children, specifically to address a previous concern received about building security at the side door exit, was created in April 2025. One of the points in this plan includes that “clear signage reminding staff and parents to close doors immediately after use”, will be posted on the doors. Evidence collected shows a sign on the side exit door that only reminds parents to “check in and out”. There is no signage posted on the door that reminds staff and parents to close doors immediately after use, therefore, this part of the action plan is not being implemented by the staff. The legislation requires that policies and procedures that are intended to guide staff in the care and supervision of children must be implemented. *This contravention is not listed as one of the allegations in the complaint.* *Correct by: June 22, 2026.*

    Observation. 1. Allegation: CCLR Section 55(1)(a) Notification of illness or injury - A parent of a child in care was not immediately informed that their child was involved in an incident that resulted in a minor injury. Findings: Unsubstantiated. Based on the information gathered, the allegation that parents were not immediately informed that their child had been involved in a minor incident is unsubstantiated. The parents were notified of the incident verbally by the staff and a meeting was arranged to further discuss the details of the occurrence, and any follow-up actions needed. Other methods used by the staff to inform the parent of an incident involving their child include phone calls, handwritten minor incident forms, and electronic messages. 2. Allegation: CCLR Section 57(2)(g) Records for each child - There is inadequate documentation that show records for a child were kept regarding any notification of a parent, emergency contact, or the medical health officer, made under Section 55 of the CCLR. Findings: Substantiated. Based on the information gathered, the allegation that there is inadequate child records kept regarding the notification of a parent, emergency contact, or the medical health office, made under Section 55 of the CCLR is substantiated. A written minor incident form for a minor incident that involved a child in care is only partially completed. There is no signature provided by the parent on this form, as per what is required in the facility minor incident policy, nor any other written record that confirms the parent’s receipt of the notification immediately following the incident. *Provide a written plan of action to correct this contravention by: June 24, 2026.* - Based on the information gathered, incidents involving a child in care on May 26/26 and June 1/26, that met the definition of "aggressive or unusual behaviour", were not reported to Licensing as required. These incidents involved the unusual behaviours of a child in care that had not been appropriately assessed in a care plan. *This contravention is not listed as one of the allegations in the complaint.* *Correct by: June 24, 2026.* - Based on the information gathered, the written “support plan” for a child in care who met the definition of a "child requiring extra support" is inadequate. The plan does not show that it has been developed in consultation with the child’s parent as is required by the legislation.   • C7.1 - Is the facility safe, sanitary, free from hazards, in good repair, and appropriate for the age and development of children?Non-critical

    Observation. 3. Allegation: CCLR Section 13(1) Environment - Ensuring a safe and secure environment for the children is not adequately maintained due to a side door routinely being left propped open. This door was seen propped open and an unhoused individual was seen standing near this unsecured entrance. Findings: Unsubstantiated. Based on the information gathered, the allegation that a safe and secure environment is not being provided for the children in care, due to a side door being propped open, is unsubstantiated. The side door automatically locks when the door closes. This side door is opened when the children are taken outside by the staff. Once the children are through the door, it is closed. 8 preschool age children at one time are taken over by an educator to the outside play yards via this side door. The door is secured after exiting. 4. Allegation: CCLR Section 16(3)(a) Play area, materials, and equipment - The height of the fence that surrounds the outside play yard is not appropriate as it is too low. The fence is shorter than the height of some of the preschool age children. Findings: Unsubstantiated. Based on the information gathered, the allegation that the outdoor play area, used by the Group Child Care (30 Months to School Age) program, is not enclosed in a manner that is suitable for the age and development of children, and will ensure the children are free from harm, is unsubstantiated. The fence measures 3 feet high on the side of the enclosure that adjoins one of the infant and toddler program yards. The other sides of the enclosure are over 3 feet high. The playground is divided into 3 separate adjoining yards. All 3 yards are fenced. The first two outside spaces are only used by the infant and toddler programs. These two spaces are located closest to the parking lot area. The third play space used by the Group Child Care (30 Months to School Age) program is located the furthest away from the parking lot area. There is a common walkway at the back of the entire outside area in order to access the individual gates for entering each separate yard. The gates are 3 feet high. An educator transitions a group of 8 children (preschool age) at one time from the facility building over to the outside play yards. The educator positions themselves near the gate after the group of children enter the yard. An outside supervision checklist for staff to follow during play outdoors is also in place and appears to be followed. - A tall wooden storage shelf, located in the common walkway area to access the gates to the outside play spaces, is not secured. The shelf has a potential risk of tipping over on to the children as they use the walkway to access the play yards. *This contravention is not listed as one of the allegations in the complaint.* *Correct by: June 8, 2026.* - The main entrance/exit door and side door emergency exit in the facility are partially blocked due to strollers being stored in these areas. In case of fire or other emergency, equipment, furniture, and supplies must not block or hamper an emergency exit at any time. *This contravention is not listed as one of the allegations in the complaint.* *Correct by: June 8, 2026.* - The laundry/storage room door, located in a main corridor which accesses the program rooms, is open. When the door is open, children have access to hazardous products stored in the laundry/storage room. There is a sign on the door that states the door must be locked at all times. *This contravention is not listed as one of the allegations in the complaint.* *This contravention was resolved by the Manager during the inspection.*
