    Observation. The information in two care plans do not have up to date information. It is important to update care plans as needed with accurate information to ensure the child is receiving the appropriate level of support by staff at all times. Submit a compliance plan to address this issue on or before May 8, 2026. One child record of 10 reviewed does not contain current information regarding parental access and custody arrangements on file. This is important to have on file so that all staff are aware of current circumstances so that the safety of the child is maintained. Submit a compliance plan on or before May 8, 2026.

  • C7.1 - Is the facility safe, sanitary, free from hazards, in good repair, and appropriate for the age and development of children?Non-critical

    Observation. The children's individual cloths for hand drying are touching each other when hung up. This poses a risk of cross contamination and the spread of germs among children. Submit a compliance plan to address this issue on or before May 1, 2026. The floor mats located in the downstairs entrance and upstairs in the hallway leading to the outdoor space have curled edges. This poses a tripping hazard. A cord is running loosely against the back of the building, inside the outdoor play space. This cord is accessible to children and poses a risk of strangulation or other injury. Submit a compliance plan to address these issues on or before May 1, 2026.

    Observation. It was alleged that the parent of a child was not notified about all incidents of serious and aggressive behaviours. Follow up to this complaint included interviews with facility staff, review of facility documentation, children's care plan and parent-staff correspondence. As a result of this follow up it has been determined that this allegation is substantiated. Submit a written compliance plan on or before July 25, 2025 regarding how this issue will be addressed. Separate to the allegations in the complaint, through follow up it was determined that many incidents of aggressive or unusual behaviour were not reported to licensing within 24 hours as required. Submit a written compliance plan on or before July 25, 2025 regarding how this issue will be addressed. Separate to the allegations in the complaint, during follow up it was determined that the licensee is not keeping a record of compliance with care plans in respect of any behavioural guidance provided to children, and its effect.     Observation. It was alleged that a staff member was yelling at children to get ready for outdoor time and to get their shoes on. Follow up to this complaint included interviews with facility staff, review of facility documentation, children's care plan and parent-staff correspondence. As a result of information provided during staff interviews, it has been determined that this allegation is substantiated. Submit a written compliance plan on or before July 25, 2025 regarding how this issue will be addressed. It was alleged that staff were not using the behaviour management strategies outlined in a child's care plan. Follow up to this complaint included interviews with facility staff, review of facility documentation, children's care plan and parent-staff correspondence. As a result of this follow up, it has been determined that this allegation is substantiated. Submit a written compliance plan on or before July 25, 2025 regarding how this issue will be addressed. It was alleged that a child stated that staff always yells at them. Follow up to this complaint included interviews with facility staff, review of facility documentation, children's care plan and parent-staff correspondence. As a result of information provided during staff interviews, it has been determined that on at least one occasion a child was yelled at by a staff member, therefore this allegation is substantiated. Submit a written compliance plan on or before July 25, 2025 regarding how this issue will be addressed. It was alleged that a child was being isolated as a form of behaviour guidance. Follow up to this complaint included interviews with facility staff, review of facility documentation, children's care plan and parent-staff correspondence. As a result of information provided during staff interviews and review of facility documentation, it has been determined that this allegation is unsubstantiated.

    Observation. The emergency plan practice drill of 2024 was not recorded. A record of these drills is important to demonstrate that this have practiced once per year as required.

    Observation. Of the child records review the following items were missing: Two were missing medical practitioner information. One was missing enrollment date. One was missing emergency contact information. An emergency contact must be someone other than the child's parents. Ensure all child files are up to date with the required information. One care plan reviewed was from 2023, with no updates, additional information or evidence that it had been reviewed since its creation. Care plans must be reviewed yearly to ensure accurate and up to date information. Yearly review also provides the chance to assess if a care plan is still needed or not. Ensure this care plan is updated and create a system to ensure that all care plans received at minimum, a yearly review. A log of all play space inspections, maintenance and repairs is not currently being kept. A log serves as a reminder for staff and also demonstrates that daily checks and consistent maintenance is taking place for the outdoor area. Create a system to meet this requirement.

    Observation. Some electrical outlets were not covered. To ensure safety ensure that all plug outlets that do not have built in safety features are covered.

    Observation. One staff record contained a criminal record check not obtained through the Licensee's organization. Ensure all CRCs are processed through the organization's ID. One staff ECE Assistant certificate has not been validated by the Licensee through the ECE BC Registry. This is required to ensure validity of all educator and assistant certificates.

    Observation. The complainant alleged that families are not notified by staff when a child has been bitten. As a result of follow up, which included review of facility documentation and staff interviews, this allegation was unable to be substantiated. Separate to the allegations made in the complaint, it was determined through follow up that the facility did not notify licensing after a child was involved in an incident described in Schedule H. Submit a report to Licensing regarding this incident and develop a plan to ensure that any items listed in Schedule H are reported within 24 hours. Separate to the allegations made in the complaint, it was determined as a result of follow up that the facility has not always logged minor incidents as required. An enhanced system is needed for staff to be able to consistently log any incidents that are not reportable. The complainant alleged that a child's care plan did not include the strategies put forth by professionals that provide additional support. As a result of follow up, which included review of facility documentation and staff interviews, this allegation has been substantiated. Care plans must include the courses of action recommended by any health care professionals. The complainant alleged that a child's care plan did not include suggestions from parents. As a result of follow up, which included review of facility documentation and staff interviews, this allegation has been substantiated. Care plans must be created in consultation with parents. Separate to the allegations made in the complaint, it was determined as a result of follow up that the facility was not keeping a record of compliance with care plan requirements. Create a system to ensure staff are recording compliance with all care plans.
