    Observation. Parent has recorded the name of a an individual who is not permitted to access to child. When Licensing asked about individual, operator wasn't aware of the instruction and did not know who the person was.

    Observation. A epinephrine auto-injector (EpiPen) belonging to child was in child's lunch bag and the lunch bag was on the seat of chair, easily accessible to children.

    Observation. Nap room is crowded with stored furniture (large desk, mattress) and other items stored around and above children's sleep mats.

    Observation. Mat used for changing diapers is in poor condition with tears in the vinyl covering (a fabric cover is being used to cover the mat which cannot simply be cleaned and sanitized after each use). A second mat, smaller than the first, is placed on top and this one is cleaned but does not cover the fabric covered mat underneath.
