Observation. Licensing audited five children's files. One child was missing their immunization record.
Corrective action. Ensure immunization record for each child was kept on file.

Observation. Licensing audited five children's files. One child was missing Doctor's Name and Phone Number.
Corrective action. Ensure Doctor's Name and Phone Number were kept on file for each child.
