Observation. The time children leave for school and the time children are picked up from school is not being recorded.
Corrective action. The manger will ensure that school age children are signed in and out as required.

- EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. Licensing observed a built up of dust in various corners of the main playroom. The manager stated she has vacuumed the floor but missed the corners.
Corrective action. Please ensure community care facility is clean and in good repair while children are in attendance

- EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. Upon inspection, licensing observed the following: - Rust on the soap dispenser in the bathroom - Fixed upon inspection - A swing set with a swing installed in the play house that did not have adequate space to swing - Materials stored in the playhouse - Plastic portion of the slide has rust and is broken with raised plastic - Swing set has paint chipping and wood rotting in more than one area - A metal top above toys used to make a roof at children's height has sharp edging around it Licensee advised children do not play in the backyard.
Corrective action. Please ensure the furniture and equipment in the community care facility are in clean and good repair while children are in attendance.

A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.
Observation. Upon inspection, the immunization status was missing for one or more children.
Corrective action. Please ensure there is a record for each child that includes the immunization status.
