  Observation. Licensing observed that 2 of 3 persons that live at the house and are over the age of 12 didn't have valid criminal record checks.

  Corrective action. Ensure that all persons on the premises that are over the age of 12 have a valid criminal record check.

  Observation. Licensing observed that an employee at the facility, had expired an criminal record check.

  Corrective action. Ensure that all employees have a valid criminal record check.

  Observation. Licensing observed that one person employed at the facility had an expired criminal record check.

  Corrective action. Ensure that all employees have a valid criminal record check.

  Corrective action. Ensure that fire drills are being practiced at least once a month.

  Observation. Licensing observed that emergency plan was not practiced during 2025.
