Observation. Licensing observed that several children did not have immunization status documented on their files.
Corrective action. Ensure for each child a record of the child's immunization status.

A licensee must keep, for each child, a record showing the following information:(i) a photograph or digital image of the child, and other information that can be used to readily identify the child in an emergency
Observation. Licensing, while reviewing children's records, observed that several children did not have a photo on file at time of inspection.
