Observation. A spray bottle containing sanitizing solution is hanging by the nozzle trigger on a shelf directly above the toilet used by children. This poses a safety risk if it were to fall as it may land directly on a child. Ensure the environment is safe and free of hazards. Provide a response to Licensing with evidence of compliance by March 9, 2026. The surface of the wooden change table used for diaper changing has several areas where paint is chipped, exposing particle board and wood. These areas are not able to be properly cleaned, which poses a potential health risk to children. Ensure the furniture, equipment and fixtures are in good repair. Provide a response to Licensing with evidence of compliance by March 13, 2026.

Observation. The outdoor play space has been changed. Licensing was not notified. Please submit a notification of change to Schedule B - outdoor play space change.
