Observation. Licensing observed a mini trampoline in the outdoor area. Licensee stated the children use it but there has been no plan submitted to Licensing for review.
Corrective action. Ensure a plan is submitted to Licensing for the use of the mini trampoline.

Observation. Licensing used the entanglement device on an outdoor composite slide that measured 3.1 feet and detected and entanglement hazard. This represents an immediate risk to the health and safety of children in care and must be corrected immediately. Licensee indicated that children will not use the slide until it is repaired or removed.
Corrective action. Ensure equipment is free of entanglement hazards in areas of uncontrolled motion.
